The Mothman Prophecies

Mark Pellington's 2002 supernatural mystery is loosely based on John A. Keel's 1975 novel of same name.

John Klein (Richard Gere) and his wife Mary (Debra Messing) are involved in a car accident when a black winged beast forces her to swerve.

She is hospitalised and diagnosed with an unrelated brain tumour, dying shortly after.

Towards the end.

John is given a message informing Mary will call at noon on Friday in Georgetown.

At a murder scene, corrupt cop Joseph Thorne finds a puzzle box and after solving it - begins to experience bizarre hallucinations.

He also makes a connection between the kidnapping of a child and a killer known as the Engineer.

While undergoing therapy, his psychiatrist reveals himself to be Pinhead, who tells him he's been in hell ever since he solved the box.  The Engineer is a manifestation of his own cruelty and the kid is the personification of his innocence.
